Heartview Foundation is a leading addiction treatment center committed to providing compassionate, patient-centered care. Our multidisciplinary team supports individuals and families in recovery through evidence-based programs and community-integrated services. 

We are seeking a dedicated Case Manager to join our team. This role is critical in supporting patients throughout their recovery by coordinating services, ensuring continuity of care, and assisting with discharge planning. The ideal candidate is collaborative, resourceful, and committed to supporting patients’ treatment goals in a culturally sensitive and ethical manner. 

 

Key Responsibilities:

  • Provide comprehensive case management, including assessment, referral coordination, and discharge planning.
  • Connect patients to essential services such as healthcare, housing, vocational rehabilitation, and transportation.
  • Maintain consistent communication with the treatment team and external partners (e.g., courts, probation officers, referral sources).
  • Document case progress per licensing and agency standards.
  • Educate and support patients and their families through the HEART program and scheduled educational sessions.
  • Attend interdisciplinary meetings and contribute to ongoing professional development.

Requirements

Qualifications:

  • Bachelor’s Degree in behavioral sciences required
  • Current CPR certification or willingness to obtain promptly
  • Valid driver’s license with a safe driving record
  • Knowledgeable in substance use disorder (SUD) and mental health recovery programs
  • Cultural and gender sensitivity in working with diverse populations

Additional Requirements: Qualified candidates must pass a background check and adhere to our drug-free workplace policy, which includes pre-employment drug and alcohol screening.
