Job Summary
The Case Manager for Mental Health and Substance Abuse will provide supportive services to individuals dealing with mental health disorders and substance abuse issues. This position is responsible for conducting assessments, creating individualized care plans, coordinating treatment and community resources, and providing ongoing support to ensure clients receive comprehensive care. The Case Manager will work closely with healthcare providers to help clients achieve their recovery goals.

Responsibilities

Perform intake assessments

Create individualized treatment plans for each client

Maintain detailed and accurate documentation of patient information and treatment plan

Maintain accurate, timely, and confidential client records in compliance with agency policies, as well as local, state, and federal regulations (e.g., HIPAA).
Prepare progress notes, treatment summaries, and other required documentation for clinical reviews and case management meetings.

Qualifications

H.S Diploma

Excellent communication

Highly organized

RADT

Schedule

Monday - Friday (11:30pm-8:00pm)

Compensation: $16.00 - $25.00 per hour
