Location: El Cerrito, CA
Schedule: Monday to Friday, 9 AM–5 PM
Salary Range: $100,000 to $120,000 annually

We’re looking for a Case Manager who thrives in a structured environment and enjoys helping people navigate care systems effectively. In this office-based role, you’ll carry a manageable caseload and help clients connect with the right services to support housing stability, wellness, and daily functioning. You'll play a lead role in planning, advocacy, and consistent follow-up.

Main Responsibilities:

• Guide individuals through service planning and access
• Coordinate external and internal resources
• Monitor outcomes and adjust support plans as needed
• Keep thorough and timely case notes
• Maintain compliance with policies and regulations

• Bachelor’s degree in Social Work, Psychology, Nursing, or a related field (Master’s preferred)
• At least 2 years of case management or client coordination experience
• Strong communication and organization skills
• Familiarity with local support systems and services
• Licensure (e.g., LCSW, LMSW, RN) is a plus
