Case Manager

Location: Oakland, CA
Schedule: Monday to Friday | 9:00 AM – 5:00 PM
Salary Range: $100,000 – $120,000 per year

We’re actively hiring a Case Manager to work onsite supporting individuals who need help navigating complex health and community systems. You’ll play a central role in coordinating services, tracking progress, and helping clients stay connected to the care and support they rely on. This is a structured office role where clear communication and follow-through matter most.

Duties:

  • Develop clear, individualized support plans
  • Link clients to local programs and track their engagement
  • Keep accurate case documentation and reports
  • Work closely with agencies, providers, and families
  • Adjust plans based on client feedback and evolving needs
  • Bachelor’s degree in social work, nursing, psychology, or similar field
  • 2+ years of experience in case or care coordination
  • Strong organizational and record-keeping abilities
  • Working knowledge of local service systems
  • LCSW, LMSW, or RN license is a plus
