Work Hours: Monday - Friday: 9 AM-5 PM

Compensation:  $100,000-$120,000 per year + benefits

Position Summary:
We are seeking a motivated and compassionate Case Manager to join our dynamic team. The Case Manager will be responsible for coordinating services, advocating for client needs, and assisting individuals in navigating healthcare, housing, and social support systems. This role is vital in helping clients achieve stability, self-sufficiency, and improved quality of life.

Job Duties:

• Conduct initial assessments and develop individualized service plans
• Coordinate care and support services across agencies and providers
• Monitor client progress and adjust plans as needed
• Advocate for client needs and assist with resource referrals
• Maintain accurate documentation and case files
• Collaborate with healthcare providers, social workers, and other professionals

• Bachelor’s degree in Social Work, Psychology, Nursing, or related field (Master’s preferred)
• Minimum of 2 years of case management or related experience
• Strong understanding of social services, healthcare systems, and community resources in California
• Excellent interpersonal, communication, and organizational skills
• Valid driver’s license and reliable transportation (may be required for fieldwork)
