Overview
Responsibilities
- Coordinate and facilitate patient care through assessment, evaluation, planning, and implementation.
- Communicate patient needs to a variety of care team members and follow up accordingly
- Manage discharge plans upon completion of treatment
- Work collaboratively with patients, families, physicians, and nurses to ensure high quality care
- Act as the patient's advocate as it relates to insurance coverage and financial assistance
- Maintain the patient's comprehensive clinical record through detailed documentation
- Coordinate with third party payors on a regular basis.
- Assist in processing laboratory orders, diagnostic tests, and more, ensuring adherence to protocols and patient safety
- Communicate patient needs and updates to healthcare team members, facilitating comprehensive care delivery.
- Advocate for patients in navigating insurance coverage and accessing financial assistance resources.
- Educate patients and their families on treatment plans, medications, and self-care techniques as needed.
Qualifications:
- 1-2 years of clinical experience, preferably in case management, or related experience. (With medical background preferred, or at least experience in medical office work)
- Proficient computer skills including experience with Microsoft Excel
- Strong written and verbal communication skills, must be bilingual in English and Spanish
- Excellent analytical skills High School Diploma or GED
- Excellent organizational and time management skills
- Familiarity with professional and technical emerging knowledge
- Problem solving skills and ability to multi-task
- Compassionate with teamwork skill
Preferred Skills:
- Bilingual in English and Spanish to serve Miami’s diverse patient population.
- Familiarity with Medicare, Medicaid, and managed care plans in Florida.
- Previous experience in wound care or similar healthcare departments.

Job Types: Full-time, Part-time
Pay: $16.00 - $17.00 per hour
Expected hours: No less than 40 per week
