Job Description - Case Manager

Position Summary

We are seeking a dedicated Comprehensive Case Manager to join our team. In this role, you will build supportive, advocacy-driven relationships with potential and active participants across our programs. You will facilitate the development and implementation of individualized plans that help reduce barriers, monitor progress, and collaborate with both internal teams and external agencies to ensure holistic support for individuals and families.

Key Responsibilities

  • Deliver comprehensive case management services, partnering closely with Intake Outreach Case Aids during assessments and plan development.
  • Develop, implement, and regularly update individualized service plans for participants and families.
  • Maintain thorough documentation of participant progress, changes, and interactions with outside agencies.
  • Foster positive, professional relationships with participants, families, internal teams, and external community partners.
  • Communicate service plans and updates clearly to all relevant team members and agencies.
  • Collaborate with the Program Manager and service team to identify and assist participants needing services.
  • Ensure compliance with program rules and proper documentation in agency databases.
  • Research and connect participants with appropriate community resources.
  • Support team members with service coverage during absences.
  • Perform other related duties as assigned.

Qualifications

  • Education & Experience:
    • Bachelor’s Degree in Psychology, Sociology, Special Education, or related field with 1+ year of case management experience,
      OR
    • Associate’s Degree with 2+ years’ experience,
      OR
    • High School diploma/GED with 3+ years’ relevant experience and/or specialized human services training.
  • Thorough knowledge of service plans, internal programs, external resources, human rights, and advocacy principles.
  • Strong skills in developing individualized plans, goal setting, technical report writing, and participant advocacy.
  • Excellent oral and written communication skills for diverse audiences.
  • Proficient in documentation, assessments, research, and meeting deadlines.
  • Ability to interpret and apply program rules and technical instructions.

Certifications & Other Requirements

  • Valid Washington State Driver’s License and proof of automobile insurance (if driving).
  • Current First Aid/CPR certification preferred.
  • Ability to pass Washington State Criminal Background Check, mandatory drug screen, and E-Verify.
  • Must not pose a direct threat to health and safety of self or others.

Physical Demands & Work Environment

  • Frequent use of hands, sitting, standing, walking, reaching.
  • Office environment with occasional travel.
  • Use of computers, office equipment, and specialized software including Microsoft Office and SET-Works Database.
