Case Manager

We are looking for a CASE MANAGER to join our team!

Hourly Rate: $45,000.00 annual salary

Located throughout New Jersey, THSM takes a multi-disciplinary approach to patient care by offering a variety of treatments and services including but not limited to: Acupuncture, Chiropractic, Physical Therapy, Occupational Therapy, Massage Therapy and Physical Medicine services. Each discipline works together with years of experience to find the best possible treatment program for our patients. Office Hours:

Monday / Wednesday / Friday: 8:30 AM to 6:00 PM

         *Office closed from 12:00 PM to 2:00 PM for lunch and shift preparations 

Tuesday / Thursday: 1:30 PM to 6:00 PM

Job Duties (Including but not limited to):

• Analyze patient schedules for accuracy with consults, report of findings, re-examinations, etc.
• Assist the Reception Supervisor in handling spreadsheets and tracking data
• Conduct patient consultations and present care plans
• Regularly communicate care plan and treatment progress with patients 
• Assist with patient questions, concerns, and complications
• Coordinate with providers and support staff
• Monitor company standards (i.e. wait times, patient education, patient outcomes, etc.)
• Find and source marketing events, patient referral sources, etc.
• Run pre-shift office meetings
• Attend and assist with in-office events and health talks
• Assist in other areas of the office when necessary (i.e. reception, therapies, etc.)

Position Qualifications:

• 2+ years of experience in a patient-facing or care coordination role
• 2+ years of experience with an electronic health record (EHR) system - EPIC preferred
• Proficiency in Microsoft Excel
• Experience in care plan and/or service-based sales
• Availability to support occasional evening or weekend events
• Experience in public speaking
