Job Description - Case Manager

BASIC FUNCTION:



Family Support Services Case Manager is to ensure the safety and well-being of native children and to assist with the enrichment of the family unit through the programs and services offered in accordance with Indian Child Welfare Act (ICWA) and under the applicable authority of the Seneca Nation Indian Child Welfare Act (SNICWA) and Seneca Nation Health System (SNHS), Family Support Services policies and procedures.


 


GENERAL RESPONSIBILITIES:


 




  1. Ensures ongoing management of cases in accordance with SNHS and Family Support Services unit policies.


  2. Accompanies and assists with child protection investigations including assessing immediate safety needs of children, developing safety plans, and making case recommendations in collaboration with respective (County) Department of Social Services agency.


  3. Develops and maintains effective and therapeutic casework relationships.


  4. Works in partnership with relevant agencies as part of a multi-disciplinary team.


  5. Assists in developing a family based service plan to reduce risk to children.


  6. Provides emergency services to the family within the scope of agency policies; which includes on-call after hours, weekends, and holidays.


  7. Coordinates services and responsibilities of other service providers working with the family.


  8. Conducts home visits for case planning and to assess for safety, suspected child abuse/neglect and will follow up according to agency policy.


  9. Attends Tribal and County Family court hearings, prepares court petitions, submission of documents, testify as required.


  10. Implements, tracks, and reports on progress related to family reunification plans for a child in care; this would include monitoring all profess with the family towards establishing individual and family goals.


  11. Supervises visits between children in care and parents/caregivers as required.


  12. Participates in the development of a concurrent permanency plan for a child in care with the family and all relevant agencies.


  13. Maintains current and accurate client case files and enters required data into the Famcare system in accordance with SNHS policy standards and reporting guidelines. Ensuring accuracy, completion and proper filing of documents within the timeframe specified by department policy.


  14. Occasional transportation and travel required. This can include travel during inclement weather.


  15. Required to complete all trainings specific to the case manager position, ie. Childcare, ICWA, Child Abuse, Goal Planning, etc.


  16. Participate in educational outreach events relevant to child welfare. Engage with the community at events & when working with other departments.


  17. Understand and implement SNICWA in accordance with the standards of ICWA, SNICWA & FSS policies and procedures.


  18. Participates and completes all mandatory staff meetings, trainings, and in-services.


  19. Follows all policies and procedures of the department, Seneca Nation Health System, and Seneca Nation.


 


KNOWLEDGE, SKILLS, & ABILITIES:





  • Excellent communication skills both orally and written.


  • Must be able to multitask and prioritize in stressful situations.


  • Ability to maintain sedentary position for computer use.


  • Must be physically fit to lift at least 30lbs.


  • Excellent time-management skills.


  • Excellent organization skills.


  • Ability and motivation to work independently and without direct supervision as well as in a team and with the general public.


 


QUALIFICATIONS:





  • Requires a Bachelor’s Degree in Social Work or related field, and two years of experience in Human Services agency.


  • Valid NYS driver’s license required.


  • Experience in a large hospital, academic medical center, outpatient health care setting, or Indian Health Service (IHS)/tribal health, preferred.
