Job Description - Case Manager


About the Position:

As a Case Manager in Continuing Care, you’ll play a pivotal role in guiding clients through one of the most critical moments of their recovery journey—the transition from treatment to real life. You won’t just create plans; you’ll create hope, stability, and the foundation for long-term success.

Schedule:

Monday through Friday 8:00 am - 4:30 pm

Your Role:

  

  • Be the Bridge to Recovery: Ensure every client leaves treatment with a powerful, personalized aftercare plan and the confidence to follow it.
  • Motivate & Persuade: Inspire clients to stay committed—even when resistance kicks in. Your words will change lives.
  • Facilitate Communication Like a Pro: Keep clients, families, providers, and referral sources in sync with proactive, timely updates.
  • Own the Details: From accurate documentation to tight scheduling, you’ll make sure nothing falls through the cracks.
  • Crisis? No Problem: Stay calm, think fast, and resolve obstacles with confidence and compassion.
  • Lead & Engage: Facilitate dynamic group sessions that educate and energize clients about the power of aftercare.
  • Be Resource-Savvy: Stay up to date on the best local resources, programs, and supports to set clients up for success.
  • Be a Client Champion: Assist with logistics like transportation, paperwork (FMLA, disability), and any other barriers that stand in the way of recovery.

Compensation:

The base rate of pay for this position is $20.00 - $22.00 per hour. Actual pay is determined based on a number of job-related factors including skills, education, training, credentials, experience, scope and complexity of role responsibilities, geographic location, performance, and working conditions.

Requirements

  • Minimum High School education with 2 Years of experience in human services, addiction treatment or related field required
  • Bachelor's Degree (B.S., B.A., or B.S.S.W.) from a four-year accredited college or university preferred
  • Must have basic computer skills including competence in software applications such as Microsoft Office suite, Adobe and use of internet applications and in using and/or connecting wireless computer hardware, the ability to project to remote devices, etc.
  • Works well independently along with the ability to work collaboratively with Clinical Treatment, Admissions, and Business Development teams at Mountainside.
  • Ability to multitask/prioritize specific caseload needs.
  • Ability to adapt to a changing environment.
  • Ability to work both independently and collaboratively as necessary.
  • Strong communication skills.
  • Excellent organization and time management skills.
