Case Manager

SHARE (Supportive Housing Assistance to Reach Excellence) is a supporting housing service designed to assist persons with serious mental illness(es) in maintaining independent housing in the community. The case managers provide the support needed in each person's home environment and in the community. SHARE provides outreach and case management services to program participants.

Position Title: SHARE Case Manager
Position Type: Full Time; 40 hours/weekly

Responsibilities:

• Perform therapeutic rehabilitative skill development
• Provide input into the CRNA and IRP
• Under supervision, coordinate ADL’s for clients in the area of independent community Living

• Bachelor’s Degree in a Behavioral Health Science
