TITLE: Case Manager

SALARY: $70,000.00 per year

SCHEDULE: Full-time position, 40 hours per week

BENEFITS: Health, Dental, and Vision insurance; Commuter stipend

About the Position

HCN provides resources and referrals, which are designed to enhance families’ sense of connection to the communities in which they live and ensure that the changing needs of the child and family are met. Our work with families is strengths-based, and both respectful of and responsive to cultural and linguistic differences in various communities.

The Case Manager is responsible for providing housing stability plans for at least 18 households per year. As part of this work, the Case Manager may be supporting families in: 

1) Searching for and securing housing, 

2) Increasing income and employability, 

3) Improving credit history and rental stability, 

4) Addressing behavioral health issues that negatively impact housing stability, and/or 

5) Accessing permanently affordable housing, including applying for placement on appropriate wait lists. 

Duties include but are not limited to those listed above in the efforts to support families in obtaining and/or maintaining stable housing.

Our comprehensive clinical services enable us to work closely with each family to understand their needs and to connect them to HCN's internal resources and our external partners. We form a trusted relationship and follow our clients over time, providing security and stability that allows them to become more stable and confident, raise healthy and emotionally independent children, and develop skills and training that will help lift them out of the cycle of homelessness.

About You!

• Obtained a Bachelor’s degree, especially in Psychology, Sociology, Social Work, or similarly related field


• Familiar with resources and communities in San Francisco, especially those dedicated to families with low income and experiencing homelessness/housing instability


• Charismatic and outgoing, and look forward to networking and outreaching with individuals and organizations


• Thrive in independent work while coming alive in a community of like-minded colleagues who share similar values


• Tenacious and able to follow through in order to best support client needs


• Willing to travel throughout San Francisco to meet with clients, families, and community partners as safety guidelines allow


• Preferably bilingual and bicultural Spanish/English
