Responsible for assessment and diagnosing of clinical nursing problems with patients, development and implementation of patient interventions and monitoring the effectiveness of those interactions in the Remote Patient Monitoring Program.1. Obtains accurate patient history and physical assessment and reviews diagnostic data in order to stratify patient’s risk level and individualize patient’s risk for heart disease, stroke, lung disease and COVID.
2. Documents initial assessment, delivery of patient care, and other pertinent information in Epic or pertinent EMR.
3. Provides educational teaching for Heart Disease to include CHF, Stroke, Covid-19 and COPD.
4. Responsible for notifying a patient’s Primary Care Provider for any alert lab values or signs and symptoms indicating heart or vascular disease or serious or alerting Covid-19 symptoms.
5. Communicates with patient and his/her family and collaborates with other disciplines for providing emotional support, teaching, and coaching support.
6. Excellent phone communication is key.
7. Assists in orientation of new personnel to unit and serves as a role model as required.
8. Serves as a positive role model of heart healthy lifestyle.
9. Contributes to a positive work climate and the overall team effort of the department and hospital, perform any related responsibility as required or directed and greet guests in accordance to our Guest Excellence principles.
10. Represents WTH with a positive and professional image while out in the community and while conversing with patients and family members.
11. Communicates information or provides assistance to ambulance personnel, law enforcement agencies, community agencies, allied health groups, or support groups in accordance with policies/procedures.
12. Performs related responsibilities as required or directed.
JOB SPECIFICATIONS:
EDUCATION:
- Completion of a diploma or an associate degree in Nursing Required.
LICENSURE, REGISTRATION, CERTIFICATION:
- Current licensure as a Registered Nurse as approved by the Tennessee Board of Nursing for practice in the state of Tennessee.
- Responsible for Continuing Education Units annually.
- BLS certification must be obtained within the orientation period, and certification must be kept current while in this position.
- Certification in area of clinical expertise by recognized authoritative agency desirable.
- Additional certifications may be required at department’s discretion.
EXPERIENCE:
- Twelve months in cardiovascular nursing preferred
- Exceptions will be reviewed on a case by case basis.
KNOWLEDGE, SKILLS AND ABILITIES:
- Skill and proficiency in the principles, concepts, and techniques of the nursing profession as normally acquired through the completion of a diploma, associate, or baccalaureate degree in nursing.
- Knowledge of policies and procedures of the hospital and completion of skills check sheet for designated areas as acquired through 3 month probationary period.
- Basic computer skills preferred. Including Word, Excel, Epic, and Cerner.
- Ability to communicate with and motivate patients and their families consistently with a positive effect. Must also display active participation in keeping current in cardiovascular/CHF/COPD/Covid and Diabetes literature and related issues.
INTERPERSONAL REQUIREMENTS:
- Interpersonal skills to perform nursing treatments and procedures and provide instruction, comfort, and support in interactions with patients and their families.
- Communication skills needed for frequent contact with unit staff, medical staff, administration, other hospital departments, and community agencies.
- Uses proper channels of communication.
- Contributes to a positive work environment and the overall team effort of the department.
DIRECTION/SUPERVISION REQUIREMENTS:
- Staff position but must have the ability to train new personnel in all aspects of the Virtual Care process.
- Acts as Charge Nurse of unit as assigned.
PHYSICAL DEMANDS:
- Ability to perform frequent moderate to heavy lifting and transporting of patients and equipment.
- Prolonged periods of standing and walking on a continuous basis.
- Ability to lift up to 50 pounds.
ENVIRONMENTAL CONDITIONS:
- May be exposed to communicable diseases and bio-hazardous materials.
OTHER JOB REQUIREMENTS:
- Works well under pressure.
- Understands patient satisfaction is a top priority and is flexible and accommodating to meet the patient’s needs and protect patient’s dignity.
- Understands and adheres to West Tennessee Healthcare’s compliance standards as they appear in the Compliance Policy, Code of Conduct, and Conflict of Interest Policy.
- Assumes responsibility for insuring continued professional growth.
- May be required to cross- train in other areas of Virtual Care.
- The employee is expected to adhere to all WTH policies and procedures.
