Job Description - Case Manager


 Full-Time | School-Based | Reports to Principal

Position Summary

The Case Manager coordinates all aspects of the special education referral, evaluation, placement, and service delivery process for students with IEPs and 504 Plans. This role ensures compliance, clear communication, and effective collaboration among staff, families, and service providers to support student success.

Key Responsibilities

  • Manage and track all IEP and 504 timelines, meetings, and compliance requirements.
  • Schedule, facilitate, and document IEP, eligibility, annual review, and 504 meetings.
  • Coordinate related services and create student schedules reflecting accurate service minutes.
  • Maintain special education records and documentation using ASPEN (SSM).
  • Serve as liaison between general education teachers, SPED staff, clinicians, parents, administration, and outside agencies.
  • Communicate regularly with teachers and families regarding accommodations, progress, and support.
  • Conduct classroom observations for students of concern or in the evaluation process.
  • Attend monthly Case Manager meetings and share updates with school leadership.
  • Attend professional development related to special education and compliance.
  • Coordinate and support implementation of IEP accommodations during state standardized assessments, ensuring compliance with testing guidelines.  
  • Support school-wide initiatives related to student services and equity.
  • Uphold ethical standards and confidentiality requirements.
  • Collaborate with administration and assign SSA to ensure program alignment and compliance.
  • Perform other duties as assigned by administration.

Requirements

 Qualifications

  • Bachelor’s degree required; Master’s preferred.
  • Valid Professional Educator License (PEL) with Special Education endorsement (or eligibility).
  • Strong knowledge of IDEA, IEPs, and 504 requirements, ISBE regulations and best practices.
  • Experience facilitating IEP/504 meetings and managing compliance timelines.
  • Strong organizational, communication, and collaboration skills.

Salary Range & Benefits

Salary Range: $65,000 – $90,000 annually, commensurate with experience and credentials.
