Job Description - Case Manager


Outreach is a Christian community service organization providing counseling, case management, youth development, older adult service, and community development. For 50 years, Outreach has changed the lives of thousands of individuals, children, youth, and families in DuPage County, IL, and we are seeking to hire a diverse staff of highly competent professional employees to pursue Outreach's mission to restore hope and provide opportunities for people to become all that God intends them to be. 

Outreach is hiring a Full-time Case Manager at Outreach Carol Stream to reduce housing instability and support long-term resident success for residents in the Carol Stream community in the Autumn Ridge Apartments and Villagebrook Apartments.

Responsibilities 

  • The Case Manager will support the recertification process, conduct benefits eligibility screenings, connect residents to employment and financial stability resources, and link residents to services available through the Outreach Carol Stream community center. 
  • The Case Manager will connect residents in rent or utility delinquency to community and emergency resources, coordinate responses to lease or eviction-related concerns, and provide targeted advocacy and crisis-oriented case management. 
  • The Case Manager will report to the Program Coordinator but will actively participate in the Outreach Carol Stream Case Management Team for routine support and staffing. 
  • Additional responsibilities include providing client transportation assistance as needed. 
  • Schedule: Monday – Friday, 8:30 am to 5:00 pm. Occasional evenings to meet client needs. 

Requirements

  • Bachelor’s degree in social work, education, or a related field. 
  • At least three to five years of direct service experience preferred. 
  • At least one year of administrative experience. 
  • Demonstrated ability in program administration and demonstrated sensitivity to the needs of individuals from low-income and culturally diverse backgrounds.  
  • A current, valid driver’s license is required along with a good driving record.  
  • A commitment to Outreach’s Statement of Christian Mission and Faith.
