Summary


We are seeking a compassionate and detail-oriented Case Manager to support adults with developmental disabilities in our lakeside community. As an advocate and QIDP-certified professional, the Case Manager will oversee a caseload of 12 individuals, helping them identify, plan, and achieve personal outcomes. The Case Manager ensures coordination across services (vocational, recreational, health, etc.) and promotes meaningful community engagement both on-campus and in the broader community. This position requires close collaboration with families, guardians, and interdisciplinary teams to provide person-centered and trauma-informed care.




The Role



  • Conduct assessments to identify each participant's strengths and needs, tailoring support plans to their personal goals

  • Develop, implement, monitor, and revise outcome-based services to ensure compliance with state and federal guidelines

  • Facilitate annual planning meetings and interim check-ins as necessary, preparing and summarizing monthly progress reports

  • Provide crisis intervention when needed, working closely with Behavioral Analysts to develop and track behavioral programs

  • Serve as the primary liaison for families, guardians, ISC, and external stakeholders, advocating for participants at medical and counseling appointments

  • Train and instruct Direct Support Persons (DSPs) in the implementation of service plans and proper documentation

  • Assist with financial and insurance concerns as necessary

  • Support participants in engaging with community activities both on-campus and off-campus

  • Attend and present cases to Behavioral Support and Human Rights committees, collaborating with Behavioral Analysts as needed

  • Follow up on participant behavioral incidents in compliance with DHS and IDPH requirements


Qualifications



  • Bachelor's degree in social services, healthcare, psychology, or a related field

  • Minimum 1 year of experience working with individuals with developmental disabilities

  • Certification as a Qualified Intellectual Disabilities Professional (QIDP) or ability to complete certification within 6 months of hire

  • Knowledge of state and federal guidelines (DHS, IDPH, HIPAA, Medicaid, etc.)

  • Valid driver's license with a good driving record

  • Must achieve CPR, First Aid, and crisis intervention certification (CPI or NCI) through the Lambs Farm training program

  • Must complete the Lambs Farm DSP program within 120 days of hire

  • Commitment to ongoing professional development, including maintaining QIDP certification

  • Physically capable of performing tasks that require bending, lifting, and walking, with the ability to lift and move up to 50 pounds to manage mobility aids and assist participants

  • Proficiency in writing, speaking, and understanding English

  • Proficiency in basic computer skills, including accurate typing, navigating websites, entering and recalling data, using word processors and spreadsheet software, managing emails, and familiarity with digital communication etiquette and techniques


Compensation


The salary for this role is $52,000 annually.
