Case Manager for Adult Services

One Full-Time Position. 40 hours per week.

Monday through Friday, traditional hours. On-call rotation.

Hiring Range: $20.16 - $21.97 / hr., depending upon education and experience.

This position will remain open until filled.

Minimum Requirements

Required:

• Bachelor’s degree in social or behavioral sciences, or a combination of four years experience and formal education in a Human Services field.

Preferred: 

• Bilingual/Bi-literate in Spanish or American Sign Language.

• Minimum of two years working with I/DD population.

• One year mental health experience.

Position Summary

The ASA Case Manager is responsible for providing person-centered care coordination for individuals with developmental and intellectual disabilities to promote a high quality of life. 

Ariel Clinical Services Mission

To provide safe, nurturing, and supportive environments for our clients so that they may have the opportunities to develop their strengths, maximize their potentials and fully participate in society.

Essential Duties and Responsibilities under the guidance of the Supervisor:

1. Establish, build, and maintain rapport with individuals, independent contractors, and other member of the multidisciplinary team in order that optimal information gathering and collective decision making may occur. These duties must be carried out in a manner that respects the rights and dignity of individuals, is driven by individual choice and promotes community inclusion.

2. Develop, implement, and monitor the appropriateness and relevance of the individual’s plans and services.

3. Assist the Supervisor in the recruitment of host homes and provider as requested.

4. Advocate for clients with pertinent agencies by being the liaison between providers for the purpose of care coordination. Maintain an effective working relationship with outside agencies. 

5. Facilitate home visits and meetings to assess the needs and progress of individuals, and intervene as appropriate. Provide mediation as needed. 

6. Document services rendered per agency Standard Operating Procedures and ensure client files are audit-ready.

7. Ensure documentation is completed and communicated in a timely manner.

8. Participate in mandatory rotation for “on call” to support our clients as requested.

9. Participate in ongoing program evaluation to ensure program effectiveness.

1. Must work closely with other Ariel Programs and offices.

2. Performs other duties as assigned.

Requirements

Required Knowledge, Skills and Abilities

1. The ability to communicate respectfully and effectively in both individual and group settings.

2. Ability to be flexible, and able to change direction rapidly with a positive attitude.

3. Ability to work independently, prioritize workload, complete a wide variety of tasks in a timely and accurate fashion.

4. Basic computer knowledge.

5. Exercise independent, ethical judgment within the scope and boundaries of the department, the Agency’s Standard Operating Procedures, and Federal, State and Local Rules and Regulations.

6. Ability to organize and present case material in a straightforward, concise and timely manner.

7. Ability to provide reliable, insured transportation for self and others through the community served.

8. Ability to be a reliable and supportive team member.

9. Ability to represent Ariel at all times in a professional and respectful manner.

10. Knowledge of community resources.

11. Ability to demonstrate consistent and reliable attendance as scheduled and requested.

Education and Experience

Required:

• Bachelor’s degree in social or behavioral sciences, or a combination of four years experience and formal education in a Human Services field.

Preferred: 

• Bilingual/Bi-literate in Spanish or American Sign Language.

• Minimum of two years working with I/DD population.

• One year mental health experience.

Certifications/Licenses

• Valid Regular Driver’s License, with acceptable driving record.

• CPR, First Aid – before contact with clients.

• CPI – within 60 days of hire.

• Initial Brief Suicide Intervention Technique (BSIT) certification

• Informed Supervision –deviant sexual behaviors training - within 60 days of hire, may not be left unsupervised with clients until this training is completed. 

• QMAP – (Medical Administration Certification) within 60 days of hire, may not provide medication to clients without this certification.

• Client Specific Training of individuals on caseload.

Physical Demands

1. This position requires that the incumbent be capable of bending, sitting, stretching, and/or reaching regularly for periods up to two hours without a break. 

2. This position requires that the incumbent have adequate hand and finger dexterity to properly perform the functions of the position.

3. This position requires lifting regularly up to 10 pounds and occasionally up to 40 pounds.

4. This position requires the ability to hear, see, smell and distinguish temperature, at a level which is adequate to ensure client and staff health and safety. Adaptive devices are acceptable.

5. This position will require daily driving of up to one hour per day and occasionally up to eight hours per day. 

Work Environment

1. Exposure to typical climate controlled office environment when in the office.

2. Exposure to all climates and weather conditions while working in the field.

3. Exposure to blood borne pathogens and infectious disease is possible, but limited.

4. At times, physical interventions may be required.

5. Exposure to hazardous chemicals is possible, but limited.
