Description

People Incorporated is seeking a Case Manager Associate to fill our Quality Health Home Specialist role at our Behavioral Health Home program. This program is team-based case management working with clients who are diagnosed with chronic mental illness. The program coordinates care and integrates social services, mental health services and primary care, to support health and wellness goals. This opportunity is a full-time hybrid position and responsibilities include providing person-centered engagement to our clients and connecting them to internal resources and referrals.

Hours: Full-Time, Monday - Friday, 8am-4:30pm

Location: Greater Twin Cities, Minnesota metro area & suburbs. Hybrid position *physical presence required*

Hiring Range: $20.00-$21.60/ Hourly, based on qualifications 

Hiring Bonus: $1,000 Hiring bonus to external candidates

Principal Accountabilities/Essential Duties:

  • Coach clients and their identified supports to increase self-efficacy, improve health management, maintain a healthy lifestyle, and improve health outcomes.
  • Work with a team based approach, supporting System Navigators, Therapists and case management team
  • Maintain regular and active contact with clients ensuring services sought are utilized and partner with client to determine effectiveness of the plan and next steps.
  • Link clients to “peer support” and “recovery support” resources in their community.
  • Maintain accurate and timely case notes on all client contacts and document all activities tied to the client.
  • Assist client/family with scheduling, transportation and support for appointments as needed.
  • Coordinate and facilitate client appointments throughout the Greater Twin Cities area.

  • Passing all background and credentialing requirements for facility and position; and
  • The ability to pass the screening required by Federal Human Services Office of Inspector General to assure that person has not been excluded from working in the Health Care Programs which receive federal funding.
  • Must have a valid and current driver’s license and auto insurance.

Education and Experience

The Qualified Behavioral Health Home Specialist must meet at least one of these qualifications:

  • Community health worker as defined in Minnesota Statutes 256B.0625 Subd. 49
  • Peer support specialist as defined in Minnesota Statutes 256B.0615
  • Family peer support specialist as defined in Minnesota Statutes 256B.0616
  • Mental health rehabilitation worker as defined in Minnesota Statutes 256B.0623 Subd. 5(4)
  • Community paramedic as defined in Minnesota Statues 144E.28 Subd. 9
  • Certified health education specialist
  • Case management associate as defined in Minnesota Statutes 245.462 Subd. 4(g) or 245. 4871 Subd. 4(j)
  • A Case Manager Associate must:
  • be at least 21 years of age;
  • have at least a high school diploma or its equivalent; and
  • meet one of the following criteria:

o have an associate of arts degree in one of the behavioral sciences or human services;

o be a certified peer specialist under section 256B.0615;

o be a registered nurse without a bachelor's degree;

o within the previous ten years, have three years of life experience with serious and persistent mental illness as defined in subdivision 20; or as a child had severe emotional disturbance as defined in section 245.4871, subdivision 6; or have three years life experience as a primary caregiver to an adult with serious and persistent mental illness within the previous ten years;

o have 6,000 hours work experience as a nondegreed state hospital technician; or

o have at least 6,000 hours of supervised experience in the delivery of services to persons with mental illness
