Overview
 


HOPE Community Services, Inc. is a leading outpatient treatment provider in south Oklahoma City.  We provide compassionate trauma-informed care to individuals whose lives have been impacted by mental health and substance use disorders. We do this by providing compassionate and evidence-based treatment, aimed at improving the quality of life for those we serve.    


HOPE Community Services, Inc. is seeking a dedicated and compassionate Case Manager for our Community Living Program to provide case management, advocacy, and support services to individuals requiring assistance with independent living and community integration. 



About the Position 


The Case Manager in Community Living is responsible for assisting clients with accessing community resources, developing independent living skills, and promoting client empowerment. The role involves coordinating services for individuals facing behavioral health challenges or co-occurring disorders, ensuring they can maintain a successful community tenure. 



About the Program 


The Community Living Program helps individuals who require frequent contact to develop skills for independent living and supports their involvement and integration into the community through case management and advocacy. The program specifically aids clients experiencing homelessness by addressing their housing, employment, and documentation needs, ensuring a pathway to stability and independence. 




Requirements 



  • Ability to obtain and maintain Case Management II (CM II) certification. 


 


Skills 



  • Detail-oriented with strong analytical skills. 



  • Strong verbal and written communication skills. 



  • Empathy and a commitment to maintaining a high standard of care. 



  • Strong organizational skills, with the ability to prioritize tasks and manage multiple projects simultaneously.
