Heartview Foundation, a leading nonprofit addiction treatment center, is expanding and looking for a compassionate and driven Case Manager for our growing family-focused services. Position Overview: As the Case Manager - Family Services at Heartview Foundation in Bismarck, you will play a pivotal role in helping individuals rebuild their lives and guiding families in their support for their loved ones who are working toward their recovery. You will work alongside an experienced, supportive team to deliver high-quality clinical services to patients in substance abuse treatment and their families. In this role, you will:

  • Provide case management for designated patient and family cases, coordinating referrals and services, and collaborate with clinical staff to assess case management needs.
  • Connect patients and families to various community services, including, but not limited to, transportation, healthcare, vocational rehab, housing, and other needs.
  • Maintain communication with the interdisciplinary treatment team and community resources for post-discharge care coordination.
  • Document and communicate appropriately with referral providers, courts, attorneys, probation officers, DOT, and social services.
  • Provide education and support to patients and family members, coordinate and facilitate family sessions and educational discussions, deliver patient education in lecture programs
  • Oversee the Heartview HEART family program, bi-weekly in the evenings, providing referrals to this program as needed.

Requirements

To qualify for this position, you must meet the following requirements:

  • Bachelor’s Degree in a behavioral science field required; candidates actively working toward completion will also be considered
  • Experience working with family systems within the behavioral health field preferred.
  • Ability/flexibility to work 1-2 evenings per week to oversee family program and coordinating family services.
  • Must have a current CPR certification or ability to obtain it within a timely manner.
  • Current and valid driver’s license and demonstration of safe driving skills.
