Case Manager/ Group Facilitator I/II (Adult Residential Treatment)

GENERAL DESCRIPTION OF THE POSITION: 

The Case Manager I at Tatum Psychology Employment Group is responsible for overseeing documentation of patient’s intake, progress, and discharge, as well as liking them with appropriate social, occupational, and educational resources as needed. The Case Manager works well with the treatment team to plan and coordinate the client’s progress through the continuum of care.  

 Schedule: (PM Shift) 2:00 pm-10:30 pm : Sunday–Thursday 

ESSENTIAL FUNCTIONS: 

1. Patient Admission & Orientation 
2. Admits new patients by completing the intake process and new patient orientation. 
3. Assessment & Treatment Planning  
4. Evaluates patients’ needs for medical, psycho-social, and psychiatric treatment. 
5. Case Management & Patient Monitoring  
6. Monitors cases by verifying patients’ attendance, observing, and evaluating progress in treatment; advocating for needed services; obtaining additional resources; providing personal support.  
7. Maintain patients’ records by reviewing case notes and documenting progress.  
8. Communication & Interdisciplinary Collaboration  
9. Communicates patients’ progress by participating in weekly interdisciplinary meetings and evaluations; disseminating results and obstacles to therapeutic team and family; identifying treatment influences.  
10. Discharge Planning & Post-Care Coordination 
11. Prepares patients’ discharge by creating discharge plans; coordinating discharge and post-discharge requirements; orienting and training family members; providing resources. 
12. Prepares reports by collecting, analyzing, and summarizing treatment and results data and trends; compiling statistics.  

ADDITIONAL FUNCTIONS: 

1. Performs other duties as may be assigned.

5. Travels occasionally during the workday and on occasional overnight stay. (Compensation for mileage, food and lodging provided.)

8. Expands and updates job knowledge through educational opportunities and professional learning.

MINIMUM QUALIFICATIONS: 

1. EDUCATION: Bachelor’s Degree in Psychology, Social Work, or related field. OR High School Diploma with Substance Use Disorder (SUD) Registration Certification, and/or Registered SUD Counselor Certification. 
2. RELATED EXPERIENCE: Zero to one (0-1) years of experience in behavioral health or a related setting. 
3. SKILLS: Strong organization and communication skills. Basic understanding of case documentation and discharge planning. Ethical behavior. 
4. OTHER EXPERIENCE / SKILLS REQUIRED: 

• Experience working with culturally diverse individuals and communities or have otherwise demonstrated a commitment to strengthening engagement of a diverse community and skill in communicating with diverse populations.  
• Familiarity with equity and/or diversity initiatives within an organization.  
• Written and oral communication skills sufficient to perform essential functions.  
• Demonstrated ability to make decisions independently and use initiative to accomplish complex assignments with general instruction and guidance.  
• Demonstrated ability to exercise judgment and integrity on the job with confidentiality, tact, and diplomacy.  
• Demonstrated ability to organize time and other resources to perform multiple tasks.  
• Demonstrated ability to complete work accurately and in a timely manner with attention to detail.  
• Demonstrated ability to work well with others and to provide effective team leadership.  
• Proficiency in word processing and database and/or spreadsheet applications.  
• Physical and mental attributes sufficient to perform essential functions.  
• Valid Driver's License  

PREFERRED QUALIFICATIONS: 

1. Bachelor's Degree in Social Work, Psychology, or a related field. 
2. Experience in dual-diagnosis or mental health treatment preferred.  
3. Proficient in English Language 
4. Proficient communicator 

WORKING CONDITIONS: 

1. Work settings vary from offices, program sites, and stakeholder locations 
2. Travel modes can include the use of company or personal transportation  

PHYSICAL JOB TASK REQUIREMENTS: 

The physical requirements checked are essential to successfully performing the duties associated with this position. 

• Sedentary work. Sedentary work involves lifting no more than 10 pounds at a time and occasionally lifting or carrying articles like docket files, ledgers, and small tools. Although a sedentary job is defined as one which involves sitting, a certain amount of walking and standing is often necessary in carrying out job duties. Jobs are sedentary if walking and standing are required occasionally, and other sedentary criteria are met.
