Position Summary
The Case Manager provides direct, trauma‑informed case management services to individuals experiencing homelessness and to patients enrolled in Recuperative Care (Medical Respite). This role serves as a critical bridge between acute medical care, housing stability, and community‑based support services, guiding clients through complex systems with compassion, structure, and advocacy.
The Case Manager conducts assessments, develops individualized service plans, coordinates care across multiple systems, and ensures clients receive the medical, behavioral health, and social supports needed to stabilize and transition successfully.

Essential Duties & Responsibilities
- Conduct comprehensive intake assessments to identify housing, medical, behavioral health, and social needs
- Develop individualized service plans with measurable goals in collaboration with clients and clinical partners
- Provide regular case management check‑ins and adjust plans as client needs evolve
- Maintain accurate, timely documentation in HMIS or equivalent systems
- Coordinate safe discharge planning for Recuperative Care clients returning to community settings
- Advocate on behalf of clients with landlords, healthcare providers, government agencies, and social service systems
Community Resource Navigation
Maintain up‑to‑date knowledge of local resources, including:
- Emergency and transitional housing
- Food, clothing, and basic needs services
- Substance use and mental health treatment
- Domestic violence and legal aid resources
- Employment and vocational programs
- Transportation and mobility services
- Build and maintain referral relationships with community partners
- Assist clients in accessing benefits such as SSI/SSDI, CalFresh, and CalWORKs
Medical Support & Recuperative Care Coordination
- Understand and communicate basic medical information relevant to client care
- Collaborate with nursing and clinical staff using appropriate medical terminology
- Monitor adherence to treatment plans and medication schedules
- Coordinate follow‑up medical appointments and assist with transportation
Qualifications and Requirements
- Bachelor’s degree in Social Work, Psychology, Public Health, Human Services, or related field
- OR equivalent combination of education and relevant experience
- 1–2 years of case management or social services experience
- Ability to work effectively with individuals experiencing homelessness, mental illness, substance use disorders, and complex medical conditions
- Proficiency with basic computer applications and electronic case management systems
- Valid driver’s license and reliable transportation
Preferred
- Experience in medical respite, skilled nursing, hospital, or clinic settings
- Familiarity with Medi‑Cal/Medicaid systems and managed care plans
- Bilingual (Spanish strongly preferred)
- Certifications such as Mental Health First Aid, CPR/First Aid, or Motivational Interviewing
- HMIS or comparable database experience

Working Conditions & Physical Requirements
- Indoor/outdoor site environment with movement between client areas and administrative offices
- Ability to lift up to 25 lbs, assist clients with mobility support as needed
- PPE required; adherence to infection control protocols
- Prolonged periods of standing, walking, and computer use
Schedule and Travel Requirements
- Full-time (40+ hours/week), Exempt
- Some evening, weekend, or on-call hours may be
