What will you do as an Intensive In-Home Case Manager? 

• As an IIH Case Manager in the Outpatient Mental Health department, you will be responsible for behavioral interventions based on the client's Person Centered Plan, psychoeducation to the client and their ecosystem, intensive case management, care coordination, crisis management, and facilitation of Child and Family Teams in collaboration with the Team Lead. 
• A typical day as an IIH Case Manager will include meeting with clients, their caregivers, natural supports, and other supportive parties to employ IIH service definition activities as outlined by NCDHHS. These activities may occur in the home, school and community as outlined in the client's treatment plan. This position requires rotation in the on-call schedule, as assigned, and flexibility with nights and weekends based on client needs.

Requirements

Minimum Qualifications/Requirements: 

• Bachelor’s Degree in human services or related field 
• 2 years of relevant professional experience
• Must have a valid Driver’s License and meet any credentialing, licensing, and privileging standards as it pertains to the department you are in. 
• All potential job candidates must pass a drug screening test, and an extensive background check is required.
