Job Description - Case Manager (ISB)


Urban Street Angels, Inc. (“USA”) is a local non-profit organization founded in 2012 with a mission to end youth homelessness in the San Diego area. At this time, USA operates within three related areas: first, we operate an emergency overnight shelter program for transitional-age youth (18 – 25 years old) living on the streets in our community; second, we operate supportive and/or recuperative housing programs for TAY seeking to leave the streets behind for good; and third, our social enterprise 8 West provides job training and transitional employment opportunities for the youth in our housing programs.

  • Status: Full-time, nonexempt, in-person
  • Hourly rate: $24-$26 (DOE)
  • Schedule: TBD
  • Location(s): San Diego, CA (downtown)

Position Summary: The Case Manager serves a key role, establishing and providing case management services for 15-30 youth clients in a shelter setting. The ideal candidate will be tasked with providing case management services within the program on an ongoing basis, but also helping to develop the program. The Case Manager will also work closely with other program team members within the organization, including peer support, housing and outreach.

Program-specific responsibilities for the Case Manager position:

  • Conducting intake assessment, and developing a road map that identifies each program participant’s unique goals and activities selected to meet their goals
  • Linking each TAY with a partnering behavioral health provider, ensuring adherence to schedules, and obtaining adequate Release of Information and other documentation at intake
  • Connecting TAY with housing or their next program as soon as possible as this is a short term shelter
  • Meeting weekly with each TAY for on-going review of program needs and progress
  • Overseeing day-to-day activities of the TAY in the program, providing input for strategic decisions that affect the personalized goals and behavioral plans for each program participant
  • Prioritizing and managing responsibilities, as evidenced by timely completion of tasks
  • Working well with others in a cooperative manner, and taking initiative as needed to solve problems
  • Interacting positively with persons of diverse socio-economic backgrounds and education
  • Fostering excellent rapport with clients and build trusting and motivating relationships
  • Recognizing mood changes or signs of behavior that indicate relapse; and always maintaining appropriate boundaries between therapeutic and social relationships
  • Setting priorities and reacting appropriately to emergency situations
  • Developing tools and systems to accommodate the success of every program participant
  • Seeking out new partnerships to provide wrap-around services to program participants
  • Attending County and partnership meetings on a regular (i.e., weekly or monthly) basis
  • Meeting with program staff on a weekly basis to discuss how TAY are progressing
  • Overseeing move-in documents with new program participants
  • Overseeing all legible, organized case note activity and communication between program staff
  • Client Intake and Progress Notes: Track ongoing interaction with clients, including phone calls, meetings, and services provided. Notes should be detailed and include dates and summaries of discussions

Job requirements:

  • Undergraduate Degree preferred (required for some programs / grants)
  • Minimum of 1 year working as a Case Manager or similar role
  • Experience working with residential programs
  • Must submit to a background check via Live Scan (with ongoing alerts), as required by specific programs, funding requirements and licensing regulations
  • Must have reliable transportation
  • Must maintain working knowledge in the following areas:  1) Empowerment, harm-reduction models, advocacy and support techniques for victims/survivors of violence, individuals with mental illness and/or substance abuse issues, etc. 2) Case management, group and individual counseling, and linkage-referral 3) Wellness and recovery concepts, Medi-Cal regulations and program documentation 4) Community resources in San Diego County and the surrounding areas 4) Relevant evidence-based practices and leading outcome-driven care
  • Bilingual a plus
