Job Description - Case Manager (IW Women's Center)

Title: Case Manager


Reports to: Program Director


Employment Status: Full-time


FSLA Status: Exempt


Pay rate: $55,000 - $65,000



POSITION SUMMARY


The Interfaith Works Women's Center (IWWC) is a fast-paced emergency shelter program for adult women experiencing homelessness. The 70-bed program provides shelter, meals, and case management services. The goal of the program is to quickly move shelter clients to housing so that homelessness is rare, brief, and non-recurring. Under the supervision of the Program Director, the Case Manager is responsible for providing comprehensive strengths-based case management services to a case load of about 18 shelter clients. Case Management consists of assessing the shelter client's needs and barriers to housing. The main goals of a Case Manager are to help the shelter clients obtain better housing, increase income, and access resources through an individualized housing plan. Case Managers must work a minimum of one evening per week.



ESSENTIAL DUTIES & RESPONSIBILITIES



  • Maintain a case load of about 20 shelter clients, developing a professional relationship with the shelter clients by engaging and building rapport.

  • Work at least one evening per week and facilitate the Community Meeting one evening every other week.

  • Assess vulnerability of shelter clients who may present with disabling conditions, a history of domestic violence or other trauma and any additional barriers to stabilization.

  • Based on assessment of needs, provide immediate intervention and support, especially if the shelter clients have emergency needs.

  • Devise individualized housing plans in collaboration with shelter clients. The Housing Plan must address goals related to obtaining better housing.

  • Refer shelter clients to needed resources, which may include medical, dental, behavioral health, vocational, and/or legal services.

  • Meet with shelter clients once a week to support their progress.

  • Maintain progress notes and enter service transactions in the Homeless Management Information System (HMIS) in a timely manner.

  • Provide crisis intervention for shelter clients who are experiencing acute behavioral health symptoms, or who are engaged in conflicts with other shelter clients.

  • Assess for suicidal and homicidal ideation, intent, and risk when needed, contacting the Mobile Crisis Team or the police for support.

  • Participate in interdisciplinary treatment team meetings to present cases, discuss appropriate interventions, and collaborate with other service providers.

  • Participate in the Coordinated Entry System calls, meetings, and trainings.

  • Other duties as assigned.



MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, ABILITIES)



  • Minimum of a bachelor's degree in a human services field such as Social Work or Counseling.

  • Two years direct service experience working with individuals experiencing homelessness who may have disabilities, including mental illness, substance use disorders, medical conditions, and cognitive impairments.

  • Understanding Trauma Informed Care, Motivational Interviewing, Harm Reduction, and the Housing First model.

  • Experience with crisis intervention, including assessing for and responding to individuals with suicidal and homicidal ideation.

  • Ability to manage multiple responsibilities efficiently.

  • Ability to work independently and with a team, collaborating with other Interfaith Works programs and departments.

  • Excellent written and oral communication skills.

  • Proficiency with Microsoft Windows and Office365, in addition to Outlook, Word, Excel, and PowerPoint.

The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this position include close vision, distance vision, and the ability to adjust focus. The noise level in the work environment is usually low to moderate.
