The Jail Reentry case manager will provide risk, mental health and substance abuse screening services to district court defendants with the goal of identifying diversion alternatives to incarceration. Services are provided primarily at the David L. Moss Criminal Justice Center, however; some services may take place at other agency locations. Services are predominantly delivered to an adult population who carry a serious mental illness, substance abuse, and/or co-occurring diagnosis. The Case Manager will assist the District Attorney in managing community bound related cases in a timely and effective manner as the matters proceed through the court system. The Case Manager is responsible for establishing cooperation and communication among the participants, attorneys, treatment providers, and other agencies and professionals involved in each case. The Case Manager will provide assessments to start treatment and jail re-entry planning. Will provide group to inmates for substance use and thinking for change.
Actively identify, locate, and engage justice-involved individuals who may benefit from services.
Conduct field-based outreach in shelters, treatment facilities, probation offices, community organizations, hospitals, encampments, and other community locations.
Develop and maintain relationships with community partners to increase referrals and service access.
Represent the program at community meetings, events, and collaborative initiatives.
