Job Description - Case Manager (licensed)

House of Mercy Inc., a sponsored ministry of the Sisters of Mercy, was founded in 1991 to provide compassionate and dignifying care for those living with HIV or AIDS. Our mission is to empower those living with HIV/AIDS in their transformation toward a more independent life. We are a small team of dedicated professionals working collaboratively to help our clients and guests achieve social/emotional well-being and economic self-sufficiency as they progress toward becoming undetectable and gaining housing stability. Guided by our CARE values, we are motivated to help end the epidemic by providing quality services for those living with HIV and the communities that support them.

Job Summary 
We are seeking a Case Manager to join our team and support the organization in our efforts to empower people living with HIV or AIDS who are houseless or at risk of becoming houseless through a range of housing and supportive services. Support the Sisters of Mercy and the House of Mercy, Inc.'s goals, philosophy, values, and mission. The role of the Case Manager is to assist with housing stabilization services and support clients' efforts to achieve economic and social/emotional self-sufficiency. The case manager will conduct assessments, develop individualized case plans, link clients with community resources, conduct home visits, and maintain an average caseload of 13 clients in temporary, transitional, and permanent housing.

To succeed as the Case Manager,  you should be able to perform each essential duty satisfactorily.

Core Responsibilities 

Provides case management and service coordination designed to assist clients in reaching optimal health and maintaining stable housing utilizing person-centered trauma-informed services. 
● Assists the Director of Programs and Case Management with completing initial psychosocial assessments for all clients 

Develop individualized care plans in collaboration with clients in areas of, but not limited to, daily living skills, money management, employment linkage, benefits establishment, linkage to substance abuse and mental health care, and all other service needs to assist clients in reaching goals.
● Assists clients in locating safe permanent housing 
● Provide crisis intervention as needed 
● Ability to identify service needs and barriers
● Maintain compliance with workflows, policies, procedures, and other activities to support the House of Mercy's model and accreditation. 
● Prepare presentations and conduct monthly outreach sessions 
● Facilitate group meetings; create and maintain an activities calendar for the psychosocial benefit of clients
● Ensures privacy and maintains security of confidential materials about programs and participants 
● Additional selected responsibilities or tasks to be performed from time to time may be required as necessary 


Knowledge, Skills and Abilities 
● Knowledge of HIV/AIDS, the social determinants of health, housing and homelessness issues, including the principles, techniques, and methods used for assessments, development of goals and activities, behavior management techniques; of cultural awareness and differences; a wide range of family and group dynamics and a wide range of intervention techniques. 
● Must be proficient in Microsoft 365 and other software programs. 
● Excellent written and oral communication skills, including writing case management notes. 
● Knowledge of resources in the community, governmental and private organizations, and protocols and expectations of various housing resources in Charlotte Mecklenburg and the surrounding area, including Gaston County.  
● Knowledge of HMIS database system and other case management software. 
● Ability to manage time, organize and prioritize work. 
● Ability to establish rapport and maintain effective working relationships 
● Ability to provide concise, accurate and timely documentation  
● Exceptional interpersonal and communication skills. 

Minimum Qualifications
●   Licensed Clinical Social Worker in the State of North Carolina 
●   Master’s degree in social work from an accredited college or university 
●   A minimum of 2 years of case management experience with chronically homeless adults, severe & persistent mental illness, and/or substance use disorder 
●   Valid driver's license and reliable transportation 
●   Ability to read and interpret documents such as leases, medical records, background checks, court documents, and written documents. 
●   Ability to write reports and develop presentations 
●   HUD CPD Part 5 Income Calculation Certificate within 30 days 
●   Experience using HMIS or obtain HMIS Certification within the first 30 days. 
●   SOAR certification or willingness to obtain in the first 90 days 
 
Physical/Environmental Demands:
Requires inside work with occasional exposure to intermittent noise and other general office conditions. Must continuously use both hands and all fingers, speak clearly, hear, see close up at less than 20" and at a distance of more than 20 feet. Requires frequent standing, sitting, and walking. Requires occasional long or irregular hours worked, especially during the production of special events. Must be able to lift, push, pull, and move equipment, supplies, etc., over (50) pounds with assistance throughout the workday. Must be able to relate to and work with ill, disabled, emotionally upset, and, at times, hostile people within our programs.

Compensation: $50,000.00 - $60,000.00 per year
