Summary/Objective Responsibilities include reviewing emails, checking phone messages, following up on referrals, reviewing documentation, ensuring proper paperwork and consents are filled out and signed, working well with other case managers, good communication skills, and working with a team.  The LVN assists with routine, direct bedside care of patients of various ages. 

Essential Functions

Maintain confidentiality about patient information.

Ability to be a role model in promoting quality care and adherence to the district Vision, Mission, and Values. 

Ability to interact with patients, co-workers, medical practitioners, and superiors in a professional manner.

Proficiency in reading medical records/charts.

Works well with a variety of age groups.

Ability to use critical thinking skills to analyze issues, develop plans for improvement, implement those plans, and monitor their effectiveness.

Requirements

Required Education and Experience

California LVN license

Current BLS required, ACLS/PALS required within 90 days of hire 

Preferred Education and Experience

Use of the Epic EHR system

 1 year of experience as an LVN or two years' experience as a CNA prior to entering the Vocational/Practical Nursing Program.
