Job Description - Case Manager -Recovery Residences

Wabanaki Public Health & Wellness (WPHW) is growing, and we are excited to be adding new people to our team!  If you value inclusivity, balance, and cultural centeredness and have a true passion for serving others, you may be a great fit for our team!  WPHW is a non-profit organization that serves four federally recognized tribes located in five communities:  the Houlton Band of Maliseet Indians, the Aroostook Band of Mi'kmaq, the Passamaquoddy Tribe at Indian Township, the Passamaquoddy Tribe at Pleasant Point, and the Penobscot Nation.  Wabanaki traditions, language, and culture guide our approach and describe the ways we live in harmony with each other and the land we collectively share.  Services are available to community members living on and off-reservation across the State of Maine.


Position Summary:


Provide community case management services primarily at our recovery homes to adults with severe and persistent mental illness or substance use disorder. Your responsibilities will involve working closely with residents (and their families) to develop personalized service plans, facilitating access to community resources, and fostering an atmosphere of trust and empowerment.  The ideal candidate is a compassionate, detail-oriented professional who can guide residents through daily challenges, coordinating with external service providers, and ensuring compliance with WPHW house and agency policies. 



Duties and Responsibilities:




    • Conduct initial assessments and intake interviews with new residents to determine their client strengths and needs. 

    • Develop client-centered individualized care and safety/crisis plans with measurable goals and objectives for each resident. 

    • Provide ongoing support and counseling to residents to address their physical, emotional, spiritual, and social needs. 

    • Assist residents in accessing community resources such as education, employment, recreational and traditional programs. 

    • Develop and facilitate life skills training sessions for residents. 

    • Coordinate and participate in team meetings with residents, guardian and other service providers and natural supports. 

    • Assist residents in accessing and utilizing community supports. 

    • Advocate and refer for services and resources for unmet needs. 

    • Assist, monitor and support residents in transitioning between services. 

    • Maintain resident (client) records and progress notes in compliance with regulatory requirements as developed by WPHW, DHHS, MARR, and MaineCare. 

    • Develop relationships with representatives from other agencies to support residents. 

    • Adhere to client confidentiality requirements and standards. 

    • Participate in all team meetings, case conferences, staff meetings, educational opportunities and clinical supervisions as directed by supervisor. 

    • Maintain professional integrity and boundaries with all providers.  

    • Work closely with House Coordinators to ensure the recovery residence is operating in a way that promotes recovery goals. 

    • Supports the recovery home staff in meeting all operational Certifications and regulations. 

    • Perform other duties as assigned.



    Education and Experience Required:



    • Education: MHRT-C Certification, and/or LDAC 

    • 1-2 years of relevant experience working with persons with cooccurring mental illness and substance use

    • Experience with Behavioral and Opioid Health Homes and Maine Accreditation for Recovery Homes (MARR), preferred 


      



      Skills and Qualifications Required:



      • Strong communication skills both verbal and written.
        Computer skills including Word and Excel.

      • Valid driver’s license and proof of automobile insurance.

      • Must pass criminal background check.
