Job Summary
Under the supervision of the Clinical Director and/or Clinical Supervisor the Case Manager position is responsible for coordination and continuity of care. Provides life skills training and development in individual and groups settings, connects clients to community resources and facilitates group. Residential Case Managers work closely within the residential interdisciplinary clinical team.
Duties/Responsibilities
• Assists clients with adjusting to residential program and preventing unplanned discharges.
• Participates in safe and solid discharge planning with client, clinician and stakeholders/natural supports.
• Coordinates care with stakeholders and family/natural supports for clients in residential program (i.e., referral source, social services, probation, etc.).
• Coordination of care with health plans and internal interdisciplinary team.
• Provide culturally appropriate modalities in support of client’s cultural preferences.
• Takes a Trauma-Informed Care approach.
• Facilitation of health promotion, life skills and appropriate psychoeducational groups.
• Provides individualized case management to residential clients addressing Social Determinants of Health which may include connecting to benefits, community resources for medical, education and/or employment needs and referrals as identified on individual service plan.
• Develop, implement, and review clinical documentation of all services provided through client records in the Electronic Medical Record (i.e., assessments, ASAM Continuum, individualized service plans, screening tools, safety plans, progress notes, discharge summary, etc.) for assigned caseload, collaborating with assigned clinician.
• Collaborates with each assigned client on individualized service plan goals throughout client’s episode of care.
• Assist in coordination of appropriate level of care for each assigned client.
• Actively participate in interdisciplinary treatment team meetings.
• Attend clinical oversight under Behavioral Health Professional, Clinical Supervisor or Clinical Manager.
• Complete accurate clinical documentation same day as date of service.
• Transport clients as needed.
• Assist in the collection of client breathalyzer readings and/or urine drug screening.
• Secure client information per HIPAA standards.
• Attends and/or provides in-service training as needed.
• Promotes mission, vision, and values of the Axiom Care program.
• Promotes inclusion through Culturally and Linguistically Appropriate Services (CLAS) Standards.
• Perform other duties as assigned.
Requirements
Required Skills/Abilities:
• Must be proficient in computer applications and electronic medical record systems.
• Proficient with Microsoft Office and related software.
• Fingerprint Clearance Card upon hire
• Current First Aid/CPR certification within 90 days of hire.
• Negative TB Test results within the past 12 months.
• Ability to communicate effectively with clients, their loved ones and professionals in both written and oral format.
• Ability to make calm sound decisions in emergency situations.
• Ability to work independently and as part of a team.
• Ability to read, analyze, and interpret human service periodicals, professional journals, technical procedures, or governmental regulations.
• Strong assessment skills.
• Strong time management skills.
• Strong case management skills.
• Working knowledge of ASAM or ASAM Continuum.
• Ability to write reports, business correspondence, and analyze policies and procedures.
• Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
• Ability to define problems, collect data, establish facts, and draw valid conclusions.
• Experience in working with diverse population and socioeconomic backgrounds.
• Experience with working with Native American population preferred.
• Working knowledge of twelve step programs of recovery and other models of self-help options in the community.
• Ability to have conversations with clients about harm reduction strategies.
• Minimal supervision regarding use of time.
Qualification Requirements
• Bachelor's degree in behavioral health-related field; or
• Associate of Arts Degree in Behavioral Health field and 2 years experience in a licensed facility; or
• High School Diploma or GED and 18 hours credit in related field and 2 years experience in a licensed facility.
• Current Arizona Drivers License and a current satisfactory 39-month Motor Vehicle Record (MVR).

Participates as an active and contributing member of a team to achieve team goals. Works cooperatively with other team members, involves others, shares information as appropriate, and shares credit for team accomplishments.
Physical Requirements:
• Prolonged periods of sitting at a desk and working on a computer.
• Episodes of standing and/or sitting facilitating groups.
• The employee must be able to lift up to 15 pounds at times.
Navajo Nation Preference:
• Preference is given to qualified Navajo Nation and Native American Applicants in accordance with Title 15 N.N.C. Chapter 7.
