Job Description - Case Manager RN


Case Manager RN – Tallahassee, FL

Location: Tallahassee, FL
Schedule: Full-Time – Day Shift (8:00 AM–4:30 PM) or Evening Shift (12:00 PM–8:30 PM)
Weekend Requirement: 1 weekend per 4-week schedule
Pay Range: $31.94 – $43.92 per hour (based on experience)
Differentials: $2.50/hr for evenings | $2.00/hr for weekends | Stackable: $4.50/hr if working both
Sign-On Bonus: $10,000 paid in 5 installments over 2 years
Relocation Assistance: Case-by-case basis

Overview:

Join the dynamic case management team at a 288-bed acute-care hospital serving Leon and Gadsden Counties. As a Case Manager RN, you will facilitate high-quality patient care by coordinating interdisciplinary treatment plans and discharge processes to ensure efficient resource utilization and excellent patient outcomes.

Responsibilities:

  • Perform assessments of psychosocial and medical needs

  • Develop and coordinate interdisciplinary care plans

  • Lead and participate in case reviews and team meetings

  • Monitor medical necessity and admission criteria

  • Communicate with third-party payers and facilitate transitions of care

  • Advocate for patients across the continuum of care

  • Identify care barriers and develop action plans for improvement

Requirements:

  • Minimum 3 years of recent Case Manager experience in an acute care setting
    (OR)
    3 years of combined experience in Med/Surg, Tele, ICU, PCU, Neuro, or ED
    (OR)
    Home Health/Insurance case management experience with at least 1 year acute care experience within the last 5 years

  • Associate Degree or Diploma in Nursing required (BSN preferred)

  • Active Florida RN license or Compact RN license with FL endorsement within 90 days

  • Case Management certification preferred
