The Source is a Joint Commission accredited addiction treatment facility that offers multiple levels of care to help men and women overcome alcohol and drug addiction. Our mission is to provide a family environment where clients get connected to their mind body and spirit, support groups, 12-step programs, and society.
The Case Manager is a professional member of the team who is a point of contact between the client and the various agencies, groups, and professionals associated with that client. The services are performed in accordance with the patient’s individualized treatment.
Responsibilities:
Provide referrals to medical or mental health professionals in the community
Provide access to legal and financial services.
Advocate for clients within the criminal justice system; supporting the client in legal or financial conflicts.
Make appointments or arrange referrals with doctors, therapists, or psychiatrists.
Link the clients with financial benefits, such as temporary disability, that can help him or her maintain personal stability while receiving treatment.
Connect the client with financial aid or benefits that can help him or her with the costs of housing, childcare, transportation, and other necessary expenses.
Coordinating the client’s appointments and obligations.
Meet with the client at appointments or consultations to act as an advocate and gather information
Help the client identify his or her personal strengths and skills.
Provide access to vocational opportunities or educational assistance so that the client can advance in his or her career
Monitor the course of treatment to make sure that the services the client is receiving are effective and appropriate
Act as supporters and motivators, engaging clients in treatment and encouraging them to strive for their goals
Assess their clients on an ongoing basis to help the treatment team determine whether they are ready to take the next step in their treatment plan.
Assist in clients in making the transition from one level of care to another as seamless as possible
Qualifications:
High School Diploma or equivalent from an accredited organization. One (1) year of relevant experience is required. Casework experience preferred.
Excellent time management, organizational, and coordination skills.
Ability to work as a team member and have management, communication, organizational and interpersonal skills.
Knowledge of addiction and mental health.
A Valid Florida’s driver’s license is required.
