Valley's CORE I program provides safe, evidence-based residential treatment for adults navigating mental health challenges, substance use disorders, trauma, and other complex life barriers. As a Case Manager, you'll play a vital role in helping clients achieve meaningful progress by providing rehabilitative services, coordinating care, supporting individualized goals, and building trusted relationships that foster lasting recovery. Schedule: Friday - Monday | 2pm - 12am | Swing Shift

Pay: dependent on certification status (see below)

• Uncertified Case Managers start at $17.50 - Uncertified Case Managers will obtain certification within the first 120 days and pay will be increased
• Certified Case Manager range starts at $19.61 - Certified CM pay is calculated based on years of certified experience.

Shift Differential: Additional $2.00/hr | when working 3+ consecutive hours between 3pm-11pm

Job Summary & Deliverables

Case Managers provide psychosocial rehabilitative services as indicated by the client’s care plan to support clients in meeting their individualized goals. Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities, and ensuring that regulatory compliance and organizational standards are met.

Essential Functions

• Provides psychosocial rehabilitative services as described by the Medicaid manual
• Supports clients in following individualized care plans and identifying case management goals
• Completes documentation on services provided within Valley’s standards
• Meets productivity expectations as assigned by supervisor
• Distributes medications as necessary
• Transports clients to appointments as necessary

Education

• High School diploma or equivalent

Licenses/Certification

• Case Manager certification (within 120 days)
• CPR certification (post hire if needed)
• Valley de-escalation certification (post hire)
• Driving positions require a minimum age of 21 and a current driver’s license

Preferred Qualifications

• Previous social services or behavioral health experience
• Bachelor’s degree in Social Services, Psychology, or related field
