Full-Time

$36,000/year

SCHEDULE: Flexible, to include some evenings/weekends; on-call duties

GENERAL STATEMENT OF DUTIES:

The Case Manager (CM) will report to the manager of the locality they are employed in. They will be expected to work within the policies and procedures of Family & Community Services, Inc., and work toward the maintenance of good service standards and professional practices. Each CM will be responsible for a case load of up to 25 veteran residents into permanent housing.

ESSENTIAL RESPONSIBILITIES:

1. Complete assessments of veteran resident and/or veteran household’s needs and to develop an appropriate support plan with the veteran, and if needed the family, with a focus on obtaining or maintaining permanent housing.

2. Assist with provision of a wide range of practical, personal, social, and general care services for the veteran resident and/or veteran household.

3. Assists veteran residents and/or veteran households in developing realistic and achievable housing plans that focus on maintaining housing stability and include measurable action steps and timetables.

4. To undertake monitoring, reviewing, and recording of the agreed upon support plan according to the changing needs of the veteran resident and/or veteran household.

5. To promote veteran resident and/or veteran household involvement by maximizing their participation in the decision-making process within the services.

6. To communicate and liaise with various agencies (e.g. Department of Veterans Affairs, Veterans Services Commission, Probation Services, Housing Authority, Landlords, Department of Jobs and Family Services, Legal and Consumer Credit Organizations).

7. To accompany veteran resident and/or veteran household to appointments with any of the above agencies as necessary.

8. Maintains regular contact with the veteran resident and/or household including childcare assessments, crisis intervention, referrals, and ongoing life skills development.

9. Assures that veteran resident future and/or veteran household housing is habitable according to established organizational standards.

10. Collects, provides and updates required data for the HMIS database.

11. Supports Social Service Worker (SSSW) and their duties in their absence or as needed

12. To provide staff support when on-call

13. To assist in providing coverage for open shifts and call offs when on-call.

14. Travels to community locations where homeless and at-risk veterans congregate to ensure effective local outreach.

15. To promote anti-discriminatory practice ensuring that no resident is discriminated against on the grounds of race, class, gender, religious beliefs, disability, or sexual orientation.

16. To be able to adapt to the environment you are working in, this could be part of a team or own initiative in a variety of settings

17. To apply Housing First, Trauma Informed Care, Harm Reduction principles and to use Motivational Interviewing, and De-escalation Techniques in working with veterans of the program as appropriate.

18. Attend meetings, trainings, and conferences as requested.

19. Participates in community-wide events promoting services for veterans.

20. To foster positive working relationships with other staff members.

21. Help SSSW, all staff, and veteran’s residents maintain up-to-date knowledge of internal fire regulations and procedures and any other emergency procedures ensures health and safety standards are maintained.

22. Complies with agency policies and procedures, COA regulations, federal and state requirements, and educational/certification/registry requirements.

23. To maintain all records and logs.

24. Other duties as assigned.

Requirements

QUALIFICATIONS:

1. Valid Ohio Driver’s License and ability to meet agency requirements for driving insurability.

2. Must be able to meet agency requirements for criminal background check (Ohio Bureau of Criminal Identification & Investigation).

MINIMUM EDUCATIONAL/LICENSURE REQUIREMENTS: Bachelor’s degree or pursuing a degree in social work and/or related field preferred. High school diploma or equivalent required.

MINIMUM EXPERIENCE REQUIREMENTS: Experience with the issues of homelessness, domestic violence, chemical dependency, mental health preferred. Experience working with veterans preferred.
