Job Description - Case Manager | Veteran Services

Description

CSPNJ, a mental health nonprofit agency, is looking for 2 Case Mangers. The Case Manager provides supportive services geared toward enabling veterans to live independently and work toward realizing their full potential. The Case Manager observes and assists veterans in retaining their housing and housing stability. This position serves Bergen, Hudson, and Passaic counites.

HIGHLIGHTS

  • Perform direct face-to-face contact with person’s served as well as documenting and coordination of care.
  • All contacts are documented in HMIS with progress notes to ensure services are properly tracked.
  • Document observations, information, and services provided to the veterans in daily shift notes, weekly progress notes, or any other emergent documentation as needed.
  • Follows team protocol for documenting and recording eligibility assessments (BVH Prioritization Tool, housing barriers assessment tool, and housing stabilization plan developed by DCA).
  • Assess veteran's daily living skills. Based on assessment, teach individuals skills in the areas of budgeting, cooking, laundry, cleaning, shopping, personal hygiene, and self advocacy, as needed.
  • Coordinate and manage services provided to veterans.
  • Conduct crisis intervention as necessary and connect veterans with appropriate emergency services.
  • Participate in community outreach and education efforts to raise awareness of homeless veterans and services.

Full-time | Benefit Eligible (Medical, free DMO dental & free vision) including 6 weeks PTO & 11 Holidays

Hourly Rate: $28.85

Requirements

  • Bachelor's degree in mental health/human service discipline OR High School Diploma / GED with equivalent work experience.
  • 2+ years in a supervisory position.
  • Knowledge of HMIS software a plus!
  • Certified Peer Specialist (CRSP) plus 1 year of related work/life experience. 
  • The ability to write clear, concise, and accurate billable progress notes. 
  • Exceptional time management skills to be able to support the needs of multiple residents. 
  • Knowledge of poverty and homelessness causes, trends, issues and solutions.
  • Detail oriented with strong organizational skills.
  • Strong judgment and problem-solving abilities to work with citizens, businesses, and others in emotional/stressful situations.
  • Strong expertise in strengths-based, solution-focused practice.
  • Strong verbal and written communication skills.
  • Knowledge and experience working with diverse cultures and ethnicities.
  • Must be proficient in MS Office products, as well as general computer literacy.
  • Ability to work with individuals with mental illness in a caring and professional manner.
  • Bilingual skills are a plus.
  • Valid NJ Driver’s License with acceptable driving record.
  • Lived Experience: CSPNJ prioritizes hiring people who use their own life experience dealing with mental health/substance use issues to inspire and support others.
