Purpose:
Responsible for the training of case managers and other positions as requested. Coordinates ongoing training and education, and assists with quality management and implementation of the SED Waiver. This position reports directly to a service coordinator. This job description is not an exhaustive listing; other duties may be assigned or implied.
Accountabilities:
· Facilitation of Wrap Around planning for Wyandotte County SED children/adolescents in averting hospitalization, or following discharge from the hospital and development of Waiver Plans of Care.
· Maintain a reduced case load of SED Waiver clients
· Responsible for oversight of the SED Waiver Plans of Cares for initial, 90 day reviews, and annual reviews.
· Coordinates with human resources to maintain adequate staffing of all case management teams and other staff as requested.
· Coordinates IT tickets, scheduling, phone set up, keys, etc. to ensure staff is ready to begin training on the first day of hire.
· Provides internal training and orientation for new case managers and other staff as requested.
· Ensures PACES staff members complete required State, Medicaid, and agency trainings.
· Coordinates monthly education opportunities for case managers and other PACES staff.
· Coordinates and provides crisis de-escalation training.
· Ensures accurate and timely completion of Waiver documents including assisting families in completing the financial application as needed.
· Represents PACES on Center committees and special projects as requested.
· Provides direct services to clients as needed.
Qualifications:
· Will possess the minimum qualifications as documented in the Kansas Case Management Program Standards for Children and Adolescents (on file).
· Ability to communicate effectively, orally and in writing.
· Ability to establish effective working relationships with staff, community agencies and clients.
· Ability to recognize and be sensitive to cultural and ethnic differences.
· Familiarity with mental health theory and practice.
· Meet or be willing to meet the minimum qualifications of a Wrap Around facilitator.
· Must be able to safely perform essential job functions, with or without reasonable accommodations.
