Job Description - Case Manager - YouthBuild DOL


Job Purpose

The YouthBuild Case Manager - serves as a bridge between school, home, and community to ensure students and families have the academic, social-emotional, and community supports necessary to thrive. This position combines individualized student coaching, family engagement, resiliency instruction, case management, and community partnership development to remove barriers to learning and promote student success, persistence, and graduation.

Duties and Responsibilities

  • Utilizes the resources from IntelliSpark, assesses students to determine pre- and post-assessment scores among the six domains of resiliency.
  • In collaboration with Associate Vice President of Strategic Innovation, implements the curriculum for identified Transformational Micro-Credentials, which support the following:
  • Confidence in the capacity to work productively with different individuals and groups toward a common goal
  • Self-Awareness and the capacity to bounce back in the face of stress, challenge, and adversity,
  • Self-efficacy and the capacity to identify needs and respond proactively.
  • Administer the TABE screening assessment to monitor student academic growth
  • Develop, implement, and regularly update Individual Service Strategies (ISS) and Success Plans aligned with participants' educational, career, leadership, and personal goals. 
  • Monitor participant progress and adjust support plans based on changing needs.
  • Serve as the primary point of contact for participants and their families. 
  • Build trusting relationships that encourage persistence, accountability, and personal growth. 
  • Coordinate comprehensive supportive services to remove barriers to participation, including referrals for housing, transportation, childcare, healthcare, mental health, employment, and other community resources. 
  • Maintain regular communication with participants, families, instructors, and community partners regarding participant progress and support needs. 
  • Empower participants and families to access available community resources and become active partners in the participant's success. 
  • Provide individual coaching focused on resiliency, goal setting, motivation, self-awareness, problem-solving, and career readiness. 
  • Support participants in developing self-efficacy, emotional regulation, communication, and life skills necessary for academic, workplace, and personal success. 
  • Monitor attendance, engagement, persistence, and overall program participation while implementing interventions as needed.

Administrative Responsibilities

? Communicates with teachers in a timely manner regarding student’s health issues or any special family needs 

? Maintains confidentiality of family and student issues 

? Assists in the development of resources of community/health services available to the families.

? Ensures that parents are receiving information that will assist them with discipline, testing, and achievement issues. 

? Empowers and encourages families to make use of all identified resources. 

? Preparing reports and documentation as required

? Reporting Case notes into the Government Performance Reporting System (GMPS) and using the WIPS system

? Participating in multidisciplinary student support meetings

Qualifications, Education and Experience

Necessary and desirable qualifications include, but are not limited to, the following:

? Must have a passion for improving educational opportunities for urban students and for building a strong, mission-driven organization. 

? Commitment to continued personal and professional growth, and a commitment to the company’s mission, vision, and growth. 

? Knowledgeable of community resources including government, health and other community agencies. 

? Demonstrated record of success. 

? Passion for improving social and educational opportunities for urban students. 

? Excellent communication, interpersonal skills, and presentation skills. 

? Ability to work effectively and efficiently with minimum supervision. 

? Passion for working with, and teaching, youth of various social, educational, and economic backgrounds and levels.

? Willingness to go “above and beyond” to support students in achieving academic goals.

? Proficiency in the use of technology. 

? Education Preferred:

  • High school diploma or equivalent 
  • Bachelor’s degree is welcomed, but not mandatory.

? Experience preferred:

  • 1 -2 years’ experience working in a childcare program and/or agency supporting families. 

Working Conditions

This job is performed in a generally clean and healthy environment. May work with challenging students at times. 

Physical Requirements

The usual and customary methods of performing the job’s functions require the following physical demands: some lifting, carrying, pushing, and/or pulling; and significant fine finger dexterity.

1:1 check in monthly as well as YouthBuild team meetings
