Case Manager – Supported Housing
Minimum Qualifications
Education: Bachelor’s degree in Social Work, Human Services, or related field
Experience: Bachelor’s degree plus two (2) years of experience providing direct services to individuals with mental disabilities or related human services experience; OR Bachelor’s degree with a one-year practicum (e.g., BSW) and one (1) year of relevant experience
Certifications/Licenses: Not specified
Work Eligibility: Must be authorized to work in the U.S.
Work Location Requirement: Onsite/community-based (regular travel to client residences required)

About the Role
Under the supervision of the Program Director/designee, the Case Manager functions as part of a team supporting individuals living in independent apartments in the community. This role assists consumers in developing and maintaining stable living, working, and social environments by coordinating access to medical, psychosocial, financial, vocational, and other essential services. The Case Manager provides crisis intervention as needed and delivers services in the field to support individuals in achieving the highest level of independent living possible.
