Your Impact

• Be the first voice of hope for new clients.

• Create personalized care plans that lead to real change.

• Connect clients to housing, therapy, jobs, and benefits.

• Partner with therapists, housing specialists, and peer support staff.

• Document client progress with accuracy and compliance.

Who Were Looking For

• Passionate about helping others & transforming communities.

• Degree in Social Work, Psychology, Human Services, OR relevant experience in case management/social services.

• Excellent communicator, organized, and resourceful.

• Knowledge of Georgias community resources a plus.

Pay & Benefits

• $17 $22/hr (based on experience).
