Case Planner

Description

 Prevention Case Planner provides intensive home- based services utilizing evidenced-based practices in partnership with enrolled individuals and families with children at-risk of placement in the foster care system. Tasks include individual and family therapy, psychosocial assessments, collaboration with an interdisciplinary team, providing psychoeducation to children and families, casework counseling, crisis intervention and linkage to community resources. Case Planner maintains records in accordance with regulatory requirements. 

Requirements

Bachelor's Level.

Background in home visiting programming.

Good communication and counseling skills with individuals.

Knowledge of community and public social service agencies.
