Job Description - Caseworker Coach II - Lead Housing Navigator

POSITION TITLE:  Caseworker Coach II – Lead Housing Navigator


DEPARTMENT:      Homeless Services


REPORTS TO:        Program Manager, La Posada


FLSA:                        Full-time, non-exempt


PAY RANGE:          $25-$27 per hour, commensurate with experience.


SCHEDULE:            40 hours per week, hours and days may vary

POSITION SUMMARY:


The Caseworker Coach II – Housing Navigator, under the supervision of the La Posada Program Manager, assists La Posada residents in establishing housing goals, coordinating services, overcoming barriers, and identifying suitable housing options. In addition to core duties, this position is responsible for leading complex case conferences, mentoring new staff, managing specialized housing partnerships, and piloting innovative approaches to housing navigation.

ESSENTIAL FUNCTIONS:



  • Assists La Posada residents in identifying their strengths and barriers and in setting attainable housing goals.

  • Provides case management and coordinates care with community partners to assist residents in accessing services appropriate to their individual needs including behavioral health care and medical care.

  • Assists clients with obtaining essential documents and completing applications to access benefits, employment, or

  • Supports residents in overcoming barriers to housing and increasing their This may include accompaniment to appointments and advocating with service providers or landlords to overcome clients’ challenges in securing suitable housing.

  • Educates clients on the current housing market, housing search options, fair housing laws, and tenant rights.

  • Documents all client contacts in Clarity HMIS and Catholic Charities tracking systems consistent with established community standards and Agency requirements.

  • Leads multi-disciplinary case conferences for residents with complex

  • Mentors and provides training to new or junior staff

  • Develops and maintains specialized partnerships with housing developers, government agencies, and local advocacy groups to expand housing opportunities.

  • Pilots new initiatives and best practices in housing navigation, reporting results and recommending improvements to program leadership.

  • Serves as a subject matter expert on housing navigation, offering guidance to other program

  • Other duties as assigned



QUALIFICATIONS:



  • Minimum of three (3) years of experience providing case management for people who are unhoused, including at least one year in a leadership or mentoring capacity.

  • Minimum one year of experience serving people living with mental illness and/or substance use

  • Associate degree in psychology, counseling, social work, or a related field or equivalent experience; Bachelor’s degree preferred.

  • Extensive working knowledge of relevant best practices including Housing First, Low Barrier Services, Safety Focused Approach, Trauma-Informed Care, Motivational Interviewing, Diversion, Homeless Prevention, Progressive Engagement, and Conflict Resolution.

  • Demonstrated experience managing complex cases and facilitating multi-disciplinary team

  • Well-developed problem-solving skills and flexible, creative thinking skills with a positive, can-do

  • Valid driver’s license and access to reliable

  • Bilingual Spanish speaker preferred but not

  • Excellent communication skills including listening and writing

  • Possess strong organizational skills and the ability to multitask and meet a demanding

  • Familiarity with or ability to quickly learn the resources and key services providers (governmental, civic, social, educational, economic, faith-based, etc.) available to help clients overcome barriers and secure housing.

  • Experience with data collection and report preparation, ability to interpret and clearly summarize data to reflect program outcomes, HMIS experience Candidate must be able to successfully complete HMIS training and obtain a Clarify user license.

  • Strong computer Ability to read and send e-mail and to utilize MS Word, Excel, Access, and PowerPoint.

  • Ability to maintain the confidentiality of current and former clients, landlords, and other matters affecting tenant relations.

  • Demonstrated ability to implement new program initiatives and evaluate their

  • Experience developing relationships with community partners and advocating for client needs at the systems level.
