Certified Medication Aide - Independence Village of Avon Lake

Position Summary:

The Certified Medication Aid is responsible for the administration of medications for residents, accurate record keeping, safe storage of medications, problem resolution and communication with residents and family. Provides resident care on occasion.

Required Experience for Medication Aide:

  • High School Diploma or GED preferred.
  • Previous Healthcare, Geriatrics or experience working with the elderly population preferred.
  • Certified Medication Aid with at least 2 years of related experience or training; or equivalent combination of education and experience.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
  • Occasionally lift and/or move up to 25 pounds.

Accountabilities for Medication Aide:

  • Timely & Accountable Attendance: Upholding to work schedules, being ready to start work on time, remaining on the job for the whole duration of shift and limited use of time off.
  • 1440 Care for residents: Understanding and leading our 1440 care standards so you and your team ensure that each resident and each family has their needs met.
  • Responsible communication with families: Communicating messages tactfully and timely with resident’s families and ensuring the message has been received and correctly understood.
  • Communicate resident changes immediately: Providing timely and detailed information to your Wellness Leader regarding any changes to a resident’s needs or care.

Other Key Responsibilities for Medication Aide:

  • Administers and accurately records the administration of medications for residents as prescribed by the physician in accordance with established policies and procedures of StoryPoint and applicable state/federal regulations.
  • Observes, records and reports to the Nurse the symptoms and conditions of residents that could be related to medication interactions, adverse drug reactions or medication side effects.
  • Determines and records effectiveness of medications in a timely and accurate manner as required by state and federal regulations.
  • Stores medications in a safe and accurate manner in accordance with established policies and procedures of StoryPoint and applicable state/federal regulations.
  • Monitors medications to ensure adequate accountability measures are taken when medications are ordered, received into the community, administered and at change of shift as required by policies and procedures of Independence Village and applicable state/federal regulations.
  • Routinely inspects, cleans and monitors equipment and supplies to ensure resident safety. Reports any equipment or supply issues to the Nurse for correction or repair.
  • Obtains, records and reports vital signs as indicated during the administration of medications.
  • Demonstrates awareness of proper infection control practice by using Universal Precautions and following policies and procedures of StoryPoint, including OSHA’s Blood-borne Pathogen Standard and StoryPoint Exposure Control Plan.
  • Communicates to residents, family, staff and others in accurate and factual manner and seeks clarification from or refers questions to Nurse when indicated.
  • Perform other duties as assigned.

General Working Conditions:

This position entails standing for long periods of time. While performing the duties of this job, the employee is required to communicate effectively with others, sit, stand, walk and use hands to handle keyboard, telephone, paper, files, and other equipment and objects. The employee is occasionally required to reach with hands and arms. This position requires the ability to review detailed documents and read computer screens. The employee will occasionally lift and/or move up to 25 pounds. The work environment requires appropriate interaction with others. The noise level in the work environment is moderate. Occasional travel to different locations may be required.
