Responsibilities:
• Assist patients with daily living activities
• Monitor and report changes in condition
• Maintain patient dignity and safety
• Communicate effectively with patients, families, and office staff

Minimum Qualifications:
• CNA Certificate
• CPR Certification
• Level II Background Screening
• TB Test (within 6 months)
• Physical – Free of Communicable Disease Statement (within 1 year)
• Own Vehicle
• Valid Driver’s License
• Auto Insurance
