Position Summary:
We are looking for a dependable and caring CNA or HHA with a positive attitude and strong communication skills. The ideal candidate should be comfortable providing hands -on care in the home setting and supporting the patient with daily needs in a safe, respectful, and professional manner.
Responsibilities:
Assist with personal care and activities of daily living
Assist with bed baths, hygiene care, and grooming
Provide wheelchair assistance as needed
Assist with safe pivot transfers to and from chair
Assist with mobility, transfers, and repositioning as needed
Provide companionship and patient support
Assist with meals and light housekeeping
Help maintain a safe, clean, and comfortable home environment
Follow the patient’s care plan and agency guidelines
Communicate any changes or concerns to the appropriate team member
Maintain professionalism and respect for the patient and family at all times
