Work hours are Monday through Friday, 8:30-5:00. This position primarily provides care to patients in Nelson and surrounding counties.
Key duties include:
· Personal Care: Assisting with hygiene, toileting, mobility, and meal preparation.
· Health Monitoring: Measuring and recording fluid intake/output, and reporting changes to the nurse supervisor.
· Documentation: Maintaining electronic medical records (EMR) and documenting services provided.
· Housekeeping: Performing light housekeeping duties in the patient area.
· Patient Environment: Maintain a clean, safe, and pleasant environment while following infection control principles.
· Professional Development: Maintaining competency and registration.
Essential Skills for Success:

· Ability to function in stressful situations
· Basic computer skills
Qualifications:
· Registered by the Virginia Department of Health Professions as a Certified Nursing Assistant
· One year of relevant experience required
· Hospice experience, preferred
· Driver’s license, personal automobile, and current automobile liability insurance
Typical Physical Demands/Working Conditions:
· Regular use of hands, reaching, stooping, kneeling, crouching, crawling, talking, and hearing. Frequent standing, sitting, climbing, and balancing. Occasional walking, tasting, and smelling.
· Ability to lift/move up to 50 pounds and push up to 200 pounds in a wheelchair.
· Effective telephone communication, keyboarding, and regular use of a computer screen.
· Frequent travel
· Must be able to work in home environments that have difficult working conditions including, but not limited to cramped space, pets, dirt, allergens, smoke, no chairs
