Job Description - Certified Occupational Therapy Assistant

Minimum requirements:



  • One year experience in acute care, extended care, rehabilitation, mental health or outpatient setting

  • Current Occupational Therapy license from the State of California

  • Current BLS/CPR certificate 



SKILLS / KNOWLEDGE / ABILITIES:


Must demonstrate technical competence, professional clinical judgment, communication skills, and sensitivity of patient and family.  Clinical skills and knowledge to successfully achieve desired outcomes with assigned patients to provide treatment to address the practical and realistic needs and goals of the patient.  Must be able to fulfill essential function of the position.



PATIENTS SERVED



This position is responsible for age-related physiological, emotional and cognitive needs of the following age of patients:  Pediatric, adolescent, adult, and geriatric.



SUMMARY OF DUTIES:


 



  1.  Completes evaluation in timely manner, including problems, goals, and plan

  2. Appropriate to the needs of the patient

  3. Reviews/considers medical history, age, precautions, functional status and prior level of function

  4. Specific to the diagnosis

  5. Assesses the need for equipment, adaptive aides, and environmental adjustments.


Establishes Goals and Treatment Plan:



  1. Based on evaluation findings

  2. Considering cultural/social aspects

  3. With respect to patient’s ability to learn

  4. Patient’s desires for care and goals are considered

  5. In terms of functional independence level

  6. Goals are objective, measurable, timely , and related to function

  7. Appropriate hierarchy of goals


Complete reassessments as appropriate




  1. Communicates effectively and documents patient treatment

  2. Informs multidisciplinary health care team of changes in patient condition and/or plan of care across the continuum of care

  3. Patient record communicates pertinent information to others in a concise timely manner.

  4. Ensures appropriate patient communication services are accessed and available such as special needs translators and interpreters.

  5. Communicates effectively with patients and family considering age-specific and cultural/spiritual needs conveying a positive professional image.

  6. Maintains current and accurate records regarding patient’s treatment and progress within department’s guidelines.

  7. Documents skilled interventions

  8. Maintains accurate daily departmental records

  9. Completes all patient documentation in a timely manner.
