Job Description - Certified Registered Nurse Practitioner



Full-time, Part-time


Description

Whether you're looking for a full 5-day week or prefer a 3–4 day schedule, we’re happy to discuss a structure that works for you.

The Certified Registered Nurse Practitioner (CRNP) will provide comprehensive and compassionate care across the lifespan of our patients. They will provide primary health care services to include: health promotion and disease prevention, education, health counseling of all kinds to patients and caretakers, clinical care during both acute and chronic illnesses, assessments for treatment of other comorbidities such as mental health conditions and drug and alcohol dependence and referrals to outside specialty providers. The ideal candidate will lead by example and work collaboratively with support staff to deliver high-quality, compassionate patient-centered care.

Employees working at least 21 hours per week (full time), are eligible for the following benefits:

The listed starting salary range is based on a full-time schedule (5 days/week). For candidates working fewer days, compensation will be prorated accordingly based on the agreed schedule.

Responsibilities:

  • Diagnose and prescribe treatment for medical conditions including STIs, HIV, HCV, and opiate addictions
  • Provide pre-exposure prophylaxis (PrEP) and post-exposure prophylaxis (PEP) as appropriate, and risk-reduction counseling
  • Perform and interpret diagnostic tests such as Pap smears, STI screenings, complete blood counts (CBCs)
  • Coordinate care and referrals for behavioral health, case management, housing, substance use services, and other social determinants of health
  • Advocate for patient-centered care using trauma-informed, culturally competent approaches
  • Perform primary care procedures such as suturing, splinting, administering immunizations, taking cultures, and debriding wounds as needed and appropriate
  • Monitor and manage co-occurring conditions, including hepatitis, and chronic diseases
  • Recommend interventions to modify behavior associated with health risks
  • Collaborate with interdisciplinary teams for continuity of care
  • Consult with or refer patients to appropriate specialists when conditions exceed the scope of practice or expertise
  • Develop treatment plans based on scientific rationale, standards of care, and professional practice guidelines
  • Provide training to other staff on site as needed
  • Document all clinical services in electronic medical record in a timely fashion
  • Participate in any clinical meetings on site or remote
  • Educate patients about self-management of acute or chronic illnesses, tailoring instructions to patients' individual circumstances
  • Provide patients with information needed to promote health, reduce risk factors, or prevent disease or disability
  • Participate in quality improvement initiatives and continuing education.
  • Keep abreast of regulatory processes and payer systems such as Medicare, Medicaid, managed care, and private sources
  • Maintain departmental policies and procedures in areas such as safety and infection control
  • Greet and acknowledge patients according to agency standards
  • Receive, record and relay messages accurately, completely and legibly
  • Speak with others using clear, professional and polite language
  • Develop and maintain positive working relationships with others, support the ACG team to reach common goal
  • Other duties as assigned

Requirements

  • Active CRNP license in the State of Pennsylvania
  • Minimum of two years clinical experience
  • Spanish language skills strongly preferred
  • Experience in women’s health preferred
  • Experience with HIV clinical medicine / addictions a plus
  • Self-motivated and reliable in unsupervised setting
  • Must be able to comfortably work with people of all lifestyles
  • Knowledge of addictions and their association with HIV disease

Work Conditions:

  • Primarily a stationary position
  • Occasional stooping, reaching, bending, kneeling, crouching
  • Occasional lifting of equipment or supplies (up to 30lbs)
  • May be exposed to infections and contagious diseases
  • Handle emergency or crisis situations
  • While performing the duties of this job, the employee is regularly required to talk and hear. The employee is frequently required to stand, walk, use hands to finger, handle or fell, and reach with arms and hands

Starting Salary Range: $125,000.00 - $145,000.00
