Children’s Case Manager: OnPoint is seeking an energetic, organized, and dedicated professional to join our team as a full-time Children’s Case Manager. In this role, you will provide essential support and services to children and youth diagnosed with autism spectrum disorder (ASD), Intellectual/Developmental Disabilities (I/DD), or Severe Emotional Disturbance (SED). Your primary responsibility will be to assist these children in developing and utilizing daily living skills, promoting community inclusion, enhancing productivity, and fostering independence in home and community-based settings. The ideal candidate will be committed to empowering children and youth to reach their full potential while providing compassionate and person-centered care.
PAY RANGE/BENEFIT PACKAGE:
Salary: starting at $55, 181.18 up to $76,150.02 annually - placement above minimum salary is based on experience.

QUALIFICATIONS:
Bachelor’s degree in social work, psychology, human services, child and family studies or related field.
Experience working with children, youth, and families.
Experience with person and family centered service planning and delivery.
Experience working independently within a strong interdisciplinary team.
Community-based service delivery experience preferred.
QIDP, QMHP, or CMHP preferred.
Familiarity with intellectual/developmental disabilities and/or autism spectrum disorders preferred.
GENERAL RESPONSIBILITIES:
Facilitate the person-centered planning process.
Monitor services including assessments, planning, and linking.
Advocate for the child and serve as a link between schools, healthcare providers, and other community agencies.
Support and empathize with children and their families to live successfully in the community and least restrictive environment for the child.
Document services according to funding, regulatory and best practice standards.
Perform duties in a variety of settings.
Establish and maintain effective relationships with consumers, families, and community professionals.
Work a flexible schedule to meet the needs of children and their families.
Utilize strength-based approaches.
Ability to structure workload while accommodating unexpected situations and client needs in a fast-paced and ever-changing environment.
Function as a member of an integrated and diverse team, offering consultation and support to team members and seeking it when necessary.
Performs other duties as assigned.
EQUIPMENT/TECHNOLOGY KNOWLEDGE:
Efficient in Microsoft Word and Excel.
Electronic Health Record (EHR).
Exceptional written and verbal communication skills.
