What is a Chronic Care Coordinator?

The Chronic Care Coordinator assures all preventive care elements are met, documented, and coordinated.

What does a Chronic Care Coordinator do?

The Chronic Care Coordinator is responsible for on-site Annual Wellness Visits, Advanced Care Planning, and assisting with chronic care management of geriatric patients that includes conducting ongoing remote care coordination.

What are some of the responsibilities of the Chronic Care Coordinator?

  • Demonstrate time management skills: Assure your work day is started at the expected start time.
  • HIPAA compliant use of computer access (need to know only) to facilitate patient care.
  • Create and maintain comprehensive, proprietary patient care plan.
  • Use proprietary care plan to guide patient interaction and direction.
  • Review individual care plans and make monthly phone calls to CCM enrolled patients.
  • Increase continuity of care by managing relationships with tertiary care providers, transitions-in-care, and referrals.

What are the qualifications for a Chronic Care Coordinator?

  • Certified Nursing Assistant preferred, but will also consider strong Medical Assistant certification or higher
  • Minimum 1-year experience in homecare, Skilled Nursing Facility or Primary Care setting
  • Minimum 1-year experience in Chronic Care Management preferred
  • Knowledge of medical terminology and disease specific interventions
  • Computer/EMR system savvy
