Community Health Worker (CHW) Supervisor

The Community Health Worker (CHW) Supervisor is responsible for supervising and supporting CHWs to ensure effective outreach, engagement, and connection to services. This role emphasizes accountability, performance outcomes, and high-quality, community-centered support.

Key Responsibilities

Supervision & Leadership

• Supervise, coach, and support CHWs to ensure productivity, quality engagement, and timely follow-through.
• Set clear expectations and monitor caseloads, outreach activities, and performance metrics.
• Conduct regular check-ins, team meetings, and performance evaluations.
• Address performance barriers and implement improvement strategies.

Performance & Results

• Track and analyze key performance indicators such as engagement rates, service linkage, follow-up completion, and documentation timeliness.
• Ensure outreach activities lead to measurable outcomes, not just activity.
• Implement corrective action plans when performance goals are not met.

Operations & Coordination

• Coordinate daily outreach activities, schedules, and coverage.
• Ensure accurate and timely documentation and data entry.
• Collaborate with internal teams and external partners to streamline referrals and service access.
• Identify system or process gaps and recommend improvements.

Community Engagement & Quality

• Support culturally responsive, community-based engagement strategies.
• Maintain professional relationships with community partners and stakeholders.
• Ensure compliance with organizational policies, program requirements, and confidentiality standards.

Qualifications

Required

• Bachelor’s degree or relevant lived experience within the communities served.
• Minimum of 2–3 years of community outreach, healthcare navigation, CHW, or related experience.
• Prior supervisory or lead experience preferred.
• Strong communication, organizational, and problem-solving skills.
• Ability to manage multiple priorities and drive results.

Preferred

• Experience working as a CHW or supervising CHWs.
• Experience in healthcare, behavioral health, or public health settings.
• Bilingual, based on community needs.
